I acknowledge that while ultrasound is generally safe, scan duration and intensity will be kept as short and low as reasonably necessary to minimize risk.

I accept that scans have limitations and may reveal unexpected findings. In such cases, the sonographer will explain the findings and seek my permission before sharing them with my healthcare provider.
